Purpose of Informed Consent

This document outlines important information about psychotherapy services provided at KLM Counseling & Healing Practices.


It is intended to help you make an informed decision about participating in counseling, in accordance with ACA and APA ethical standards.


Please read carefully and ask any questions you may have.

Nature of Psychotherapy

Psychotherapy involves exploring thoughts, emotions, behaviors, and experiences to improve mental health and overall well‑being.


Therapy may include discussing difficult topics, learning coping skills, processing past experiences, and identifying patterns.


You and your therapist will work collaboratively to determine goals and the pace of treatment.

Potential Risks and Benefits

Potential benefits include improved emotional health, increased coping skills, better relationships, and enhanced self-awareness.


The outcome depends largely on the client’s willingness to participate and engage in the therapeutic process, which may sometimes result in discomfort. Remembering unpleasant events and becoming aware of feelings attached to those events can bring on intense feelings of anger, depression, anxiety, etc. There are no miracle cures. The therapist cannot promise that a client’s circumstances will change. The therapist can promise to provide support, understanding, care and concern and help the client clarify what they want for themselves.

Confidentiality

All information shared in therapy is confidential and protected by state and federal law.


Information may only be released with your written consent, except in the following legally mandated situations:


• Risk of harm to yourself or others

• Suspected abuse or neglect of a child, elderly adult, or vulnerable adult

• Court order or legal requirement

• Situations requiring disclosure to ensure public safety


Your therapist will discuss limits of confidentiality with you if these situations arise.

Professional Boundaries

Therapists follow ACA and APA ethical codes regarding professional boundaries.


Dual relationships (e.g., friendships, business relationships) outside the therapeutic relationship are not permitted.


Communication outside of sessions will be limited to scheduling and administrative matters unless otherwise discussed.

Telehealth Sessions

Telehealth services may be offered when clinically appropriate and mutually agreed upon.


Telehealth requires a private location, reliable internet connection, and HIPAA-compliant communication systems.

Appointments, Scheduling, and Fees

Standard therapy sessions last approximately 50-60 minutes unless otherwise arranged.


Your session fee is: $125 per session.


Late cancellation policy:


• First late cancellation (<24 hours): No fee

• Second late cancellation (<24 hours): $65 fee

• Third late cancellation: Full session fee ($125)

• Continued late cancellations may result in termination of services


Payment is due at time of service unless otherwise arranged.

Client Rights

• You have the right to ask questions about your treatment at any time.

• You have the right to refuse therapeutic techniques or interventions.

• You may request a referral to another therapist at any time.

• You may discontinue therapy whenever you choose.

• You have the right to access your records within legal and ethical limits.

Client Responsibilities

• Attend sessions consistently and arrive on time.

• Participate actively in the therapy process.

• Communicate openly with your therapist about concerns or changes.

• Ensure payment is made in accordance with office policies.

Crisis and Emergency Procedures

KLM Counseling does not provide 24-hour emergency services.


If you are experiencing a mental health emergency, call 911 or go to the nearest emergency room.


You may also contact local crisis hotlines or 988 (Suicide & Crisis Lifeline).

Purpose of Telehealth

Telehealth counseling involves delivering mental health services through secure electronic communication technologies when in-person sessions are not possible or preferred.


This consent outlines your rights, responsibilities, and the limitations of telehealth services in alignment with ACA and APA ethical standards.

Nature of Telehealth Services

Telehealth services may include video sessions, phone calls, or secure digital communication.


Sessions will be conducted using HIPAA-compliant platforms to maintain client confidentiality.


Telehealth may be used temporarily or as an ongoing form of treatment depending on your needs and clinical appropriateness.

Potential Benefits of Telehealth

• Increased access to care from the comfort of your home.

• Reduced travel and scheduling barriers.

• Continuity of care during illness, travel, or unexpected circumstances.

Potential Risks and Limitations

• Technology issues such as poor internet connection or interruptions.

• Privacy risks if sessions are conducted in shared or unsecured environments.

• Telehealth may not be suitable for all clinical needs or high‑risk situations.

• Although security measures are used, telehealth may carry additional cybersecurity risks.

Confidentiality and Privacy

Confidentiality in telehealth follows the same federal and state laws that apply to in‑person counseling.


HIPAA-compliant software will be used for all video sessions.


Clients must take reasonable steps to ensure privacy during sessions (e.g., using a quiet room, headphones).


Records from telehealth sessions are maintained in the same manner as in‑person records.

Technology Requirements

You will need a stable internet connection, a private space, and a device with audio and video capability.


You agree to notify your therapist if technological issues interfere with your ability to participate safely or effectively.

Emergency Procedures

Telehealth is not appropriate for emergency situations.


At each session, you must provide your current physical location.


You must provide an emergency contact who can be reached if safety concerns arise.


If you experience a crisis or life‑threatening emergency, call 911 or go to the nearest emergency room immediately.

Client Rights

• You may withdraw consent for telehealth at any time.

• You may request in-person appointments when available.

• You may ask questions about any aspect of your treatment.

Client Responsibilities

  • Ensuring a private and safe environment for each session.

  • Not participating in telehealth sessions while driving or in unsafe conditions.

  • Providing accurate and complete information to your therapist.

  • Using secure devices and internet connections when possible.

  • To maintain confidentiality, client will not share the telehealth appointment zoom link with anyone unauthorized to attend the appointment.

Fees and Policies

Telehealth sessions are billed at the same rate as in‑person sessions unless otherwise discussed.


Cancellation and no‑show policies apply equally to telehealth appointments.
